    Apply for this insurance online, or by mailing/faxing a PDF application.
  2. Select the “Enroll Now” button above to launch the online application through Prudential’s secure portal.

    OR

    Select the “Print an Application (PDF)” button above to print a PDF version of the application. Then, complete and mail/fax to the address at the top left corner of the document.
  3. Once approved, you will receive your insurance verification kit and hard copies of your certificate of coverage will be mailed to you for your records.

* Limited benefits for the first three years: upon the death of the member, the beneficiary would receive $500 during the first year of coverage, 25% of the coverage amount during the second year, 50% in the third year and 100% thereafter. Coverage terminates at age 80. In case of accidental death at any time, the policy pays 100% of the benefit amount. Benefit reduces at Age 60.

** Insurance death proceeds are generally received income tax free under IRS Section 101(a).
